Privacy Policy

Your Privacy: Our Commitment to Your Privacy & Confidentiality

At ODC, we recognize that the health information of our patients deserves to be treated with respect and sensitivity and our patients’ privacy must be protected. Access to your personal health information is available to those who need to know to provide care, including: physicians, nurses, technicians, therapists and other health professionals. Furthermore, when it comes to collecting and using your personal health information, we take measures to ensure the privacy and confidentiality of the information is protected.

PRIVACY POLICY

ONTARIO DIAGNOSTIC CENTRES recognizes the importance of protecting the privacy of sensitive personal
information, in particular, the personal health record information, which we gather as a component to the
diagnostic services we provide to our patients. We are committed to collecting, using, and disclosing this
information in the most responsible fashion possible, and only to the extent necessary with respect to the
services we provide, or have provided in the past.

ONTARIO DIAGNOSTIC CENTRES, in conjunction with the Privacy Information Officer, is responsible for the personal information collected and held during the course of the relationship with our patients. To ensure accountability a Privacy Policy has been drafted which is adhered to by all team members, and enforced by the Privacy Information Officer. This policy outlines how personal information is managed and safeguarded, and the circumstances under which personal information may be disclosed.

WHAT IS PERSONAL INFORMATION?

Personal information is any information that can possibly reveal the identity of an individual, or can permit
the deduction of the identity of an individual. Personal information includes any health-related information
collected during registration of a patient, produced during the course of a diagnostic procedure a patient
undergoes at ONTARIO DIAGNOSTIC CENTRES, or is otherwise submitted to our facility by a patient or their
referring physician. Business information however (title, address, phone number, etc.) is not included in our
Privacy Policy, and is not covered under the regulations of the Personal Health Information Protection Act
(P.H.I.P.A).

WHEN WOULD PERSONAL INFORMATION BE COLLECTED, USED, AND/OR DISCLOSED?

ONTARIO DIAGNOSTIC CENTRES can use, collect, and/or disclose personal information for the following
reasons:

A. TO ENSURE QUALITYHEALTH-CARE:
  • By delivering quality diagnostic imaging services to all of our patients.
  • By providing the continuous high quality service you, and other patients have come to expect from ONTARIO DIAGNOSTIC CENTRES.
  • By assessing your health care needs. By allowing us to efficiently follow up your case for additional or repeated testing, ongoing treatment and care.
  • By enabling us to contact you directly and maintain communication with you (i.e. in order to distribute health care information directly to you, or to book and confirm appointments).

B. FOR BILLING AND ADMINISTRATION PURPOSES:

  • In order to directly bill you for the diagnostic service(s)rendered.
  • To complete and submit remittance claims to O.H.I.P, or to a third-party agency (such as a private
    health insurance plans or a community centre) thereby ensuring payment for the diagnostic
    service(s) rendered to you by ONTARIO DIAGNOSTIC CENTRES.
  • To determine or verify the eligibility of individuals for health care benefits.
  • To collect unpaid accounts and to process payments in general.
  • For administration and management decisions (such as planning resource allocation, assessing
    reporting systems or an overall facility evaluation).
  • To permit potential purchasers or advisors to evaluate ONTARIO DIAGNOSTIC CENTRES in
    preparation of a sale or audit.
  • To deliver patient health-care charts and records to our insurance company so that they can assess
    liability and quantify damages in the event of a lawsuit.

C. FOR RESEARCH PURPOSES:

  • For health-care research, public health surveillance, and statistical data analysis purposes by an
    appropriate group or individual.
  • For teaching, demonstration, and reference purposes on an anonymous basis.
  • To help determine if you are an appropriate candidate for a particular research study.

D. FOR YOUR PHYSICIAN:

  • To advise you and your physician (and their consultants) with respect to health related matters, as
    well as to render a diagnosis and treatment recommendations or options.
  • To communicate with your physician or other health-care providers and organizations when
    required, for your continued health-care.

E. FOR GOVERNMENT SANCTIONED ORGANIZATIONS:

  • In assisting ONTARIO DIAGNOSTIC CENTRES in its pursuit to comply with all legislated statutory
    requirements.
  • To comply with legal and regulatory requirements, including but not limited to the delivery of
    patient health-care charts and records to the College of Physicians and Surgeons of Ontario
    (C.P.S.O), the Ministry of Health and Long-Term Care (M.O.H.L.T.C), or others, as stipulated in the
    Independent Health Facilities Act, or the Healing Arts Radiation Protection Act (and successor
    legislated Acts) in a timely fashion.
  • To comply with any other agreements or undertakings entered into, with C.P.S.O. or the
    M.O.H.L.T.C.
  • To prepare materials for the Health Professions Appeal and Review Board.
  • To generally comply with the law when such a purpose is identified before, during, or after the time
    such information is collected.

Cookies: Like most websites, ODC uses small pieces of information called “cookies” that are stored on the
hard drive of your computer. Cookies let us “remember” information about your preferences and allow you
to move within areas of our site quickly and easily. These cookies contain anonymous, non-personal
information.

Information About Your Visit: In order to improve access to our website and determine what information
visitors access most frequently, we may track statistics, such as IP address, browser type, domain name,
access times and referring website addresses.

Third-Party Sites: This site contains links to external websites. UHN is not responsible for the privacy
practices or the content of any third-party website.

Updates: We may update these Terms and Conditions from time to time. The most current version will
always be posted to this website.
